Building on our mission to keep disabled loved ones safe at home, TNT Multi-Care Foundation is committed to providing compassionate, community-based care and resources for individuals with intellectual and developmental disabilities, brain injuries, mental health challenges, aging needs, and veterans transitioning to civilian life. We strive to fill critical gaps in services and ensure every individual we serve has the opportunity to live a full, dignified, and supported life.
Our vision is a society where mentally challenged individuals, veterans, and the elderly thrive with dignity, compassion, and empowerment.

Origin Story
TNT Multi-Care Foundation was born from the heart of Yolanda Jones, a devoted mother and caregiver whose life was forever changed by a tragic car accident that left her young daughter, Tina, with a traumatic brain injury. From that moment on, Yolanda became not only Tina’s mother but also her full-time advocate, caregiver, and protector.
Raising a child with such high needs while working full time wasn’t easy. Navigating government programs like CLASS and HCS was overwhelming and often disheartening. There were days when Yolanda felt invisible in the system — but she made a personal promise: Tina would never end up in a facility. She would be cared for at home, with dignity, love, and unwavering support.
Yolanda kept that promise, caring for Tina every single day until her passing at age 37. In honoring Tina’s life and legacy, Yolanda founded TNT Multi-Care Foundation — named in Tina’s memory — to help other families avoid the isolation, burnout, and resource gaps she once faced.
TNT was created to support caregivers, advocate for vulnerable populations, and ensure that no family has to walk this journey alone. We exist to address the systemic gaps in care, offering support not only to individuals, but also to their families, veterans, and others navigating overwhelming challenges in a complex system.
Our foundation is grounded in the belief that every person deserves to be seen, supported, and empowered to live with dignity — as we carry forward Yolanda’s mission to ensure that disabled, elderly, and vulnerable individuals can remain safe and well cared for in their own homes — just like Tina.
